An International Vaccination Health Certificate or a Pet Passport showing your pet has been inoculated and is free from diseases will grant your pet permission to be imported into Spain without quarantine. The health certificate must be issued no more than 15 days prior to bringing your pet into the country, and must state: 
1.  That your pet shows no signs of disease and has been examined before leaving the country.
2.  The identification and origin of your pet.
3.  That your pet has been vaccinated against rabies.
4.  You must also have an ID chip implanted in your pet.  Be sure it is the European version as the U.S. and UK versions cannot be read by European scanners.

Animals must be accompanied by either its owners or a representative, and must have written certification that the cat or dog has been under the owners' supervision for at least 3 months before entering Spain.

If you are flying to Spain, check with your air carrier before planning when and how to bring your pet to Spain. Some carriers will not transport pets in the luggage compartment during the summer months due to the risk of harm to the pet from the heat.
